Recognizing Patterns
Human societies are organized through hierarchical relationships. Parents guide children. Teachers educate students. Physicians care for patients. Managers lead employees. Judges preside over court proceedings. Military officers command units. Across every culture and throughout recorded history, these relationships have enabled human beings to transmit knowledge, coordinate responsibilities, allocate resources, make collective decisions, and accomplish tasks that no individual could achieve alone.
Although these relationships differ in purpose, they share an essential characteristic: each depends upon learning. Parents teach children. Teachers guide students. Experienced physicians train residents. Senior employees mentor junior colleagues. Military officers prepare future leaders. Every hierarchy exists, at least in part, to transfer knowledge and improve collective performance across generations.
Yet careful observation reveals another recurring pattern.
Some hierarchical relationships consistently promote learning. Questions are welcomed. Mistakes become opportunities for improvement. Existing assumptions are examined rather than defended. Individuals contribute ideas freely, and organizations adapt as new knowledge emerges.
Others develop very differently. Questions gradually disappear. Existing assumptions remain largely unchallenged. Errors are concealed rather than examined. Individuals learn that disagreement carries risk, and organizational learning slows despite the sincere intentions of those working within the system.
These contrasting supervisory patterns appear not only within individual organizations but across medicine, education, business, government, the military, the legal system, and families.
The recurrence of these observations suggested a simple but profound scientific question.
Why?
Learning Begins with Patterns
Throughout the history of science, discovery has begun with the recognition of recurring patterns.
Ancient astronomers observed predictable movements of the stars long before understanding gravity.
Naturalists recognized similarities among living organisms centuries before evolutionary theory explained them.
Physicians observed recurring patterns of disease long before microorganisms were discovered.
Mathematicians recognized numerical relationships before expressing them through equations.
Across disciplines, scientific progress follows a remarkably consistent sequence:
A pattern is observed.
A question is asked.
The search begins for the rule responsible for producing that pattern.
Once the rule is discovered, the pattern becomes understandable. Often it becomes predictable. In many cases, it also becomes modifiable.
Human learning follows this same process.
Learning as the Search for Rules
Learning is commonly described as the acquisition of knowledge.
A more fundamental description is that learning consists of discovering the explanatory rules responsible for recurring patterns.
Children discover the grammatical rules governing language.
Scientists search for the physical laws governing nature.
Engineers identify the principles governing machines.
Physicians uncover the physiological mechanisms responsible for disease.
Mathematicians express numerical relationships through equations.
Learning therefore involves much more than accumulating information.
It involves discovering the explanatory rules that organize observations into coherent understanding.
This insight also reveals why recognizing the limits of one's own knowledge is indispensable to learning.
People search for better rules only when they recognize that their current explanation may be incomplete.
If we believe we already know the rule responsible for a pattern, we stop looking.
Curiosity depends upon uncertainty.
Discovery begins with intellectual humility.
Existing Explanations
The recurring supervisory patterns observed across human hierarchies have attracted scholarly attention for decades.
Leadership researchers have examined supervisory styles.
Organizational scientists have investigated institutional culture, communication, incentives, and systems thinking.
Psychologists have explored cognition, unconscious bias, motivation, and decision-making.
Political scientists have studied authority, legitimacy, and democratic governance.
Educational researchers have examined reflective practice and collaborative learning.
Collectively, these disciplines have transformed our understanding of human organizations.
Each has identified important rules governing particular aspects of organizational life.
Yet the recurring supervisory patterns themselves persisted across institutions that differed profoundly in profession, purpose, governance, and culture.
The pattern appeared larger than any single disciplinary explanation.
An Observation from Practice
My interest in this question emerged gradually through clinical medicine, graduate medical education, organizational leadership, quality improvement, and public health.
Across these experiences, I encountered individuals who shared remarkably similar aspirations.
Teachers wanted students to learn.
Physicians wanted patients to receive excellent care.
Managers sought organizational improvement.
Public officials sought to serve their communities.
Yet the supervisory environments they created often differed dramatically.
Some supervisors consistently welcomed questions they could not immediately answer.
Others became increasingly certain that they already possessed the correct answers.
Some viewed disagreement as an opportunity to improve understanding.
Others experienced disagreement primarily as a challenge to authority.
These differences appeared repeatedly across professions that otherwise shared almost nothing in common.
Eventually, the question itself changed.
Rather than asking why organizations differ...
I began asking why remarkably similar supervisory belief–behavior systems emerge across organizations that are otherwise so different.
A Different Kind of Scientific Question
Scientific inquiry advances by identifying the rules responsible for recurring patterns.
The supervisory belief–behavior systems observed across human hierarchies constituted such a pattern.
Leadership theory described many of these patterns.
Organizational science explained important institutional influences.
Psychology illuminated numerous aspects of human cognition.
Political science clarified democratic and authoritarian systems of governance.
Educational research explained characteristics of effective learning environments.
Each discipline contributed an essential part of the explanation.
Yet one question remained.
What underlying rule consistently gives rise to these recurring supervisory belief–behavior systems?
This question differed fundamentally from asking which leadership style is most effective or which organizational structure produces the best outcomes.
Instead, it asked whether a more fundamental cognitive rule might explain why remarkably similar supervisory belief–behavior systems emerge across human hierarchies regardless of institutional context.
The Beginning of an Investigation
Every scientific investigation begins with an observation that existing explanations cannot fully resolve.
For me, that observation was the remarkable recurrence of similar supervisory belief–behavior systems across medicine, education, business, government, and countless other forms of human organization.
Those recurring supervisory systems constituted a pattern.
Like every scientific investigation before it, the next step was not to invent a new theory.
The next step was to search for the rule responsible for producing the pattern.
That search required beginning somewhere more fundamental than organizations themselves.
Organizations do not learn.
People do.
Understanding how human beings discover the rules that explain patterns—and why they sometimes stop searching for better explanations—became the starting point for everything that followed.
The search for that rule became the work of the next twenty-five years.
Continue the Investigation
Chapter One has introduced the central observation that motivated this research: remarkably similar supervisory belief–behavior systems emerge across diverse human hierarchies despite profound differences in organizational purpose, profession, and culture.
The next chapter examines how previous generations of scholars have attempted to explain these patterns and identifies the remaining scientific question that ultimately redirected this investigation toward learning, human cognition, and supervisory epistemology.
