The Puzzle of Human Hierarchies
Scientific discovery often begins with a simple observation: the same pattern appears repeatedly in places where no one expected it.

Recognizing the Pattern
Human societies are organized through hierarchical relationships.
Parents guide children. Teachers educate students. Physicians care for patients. Managers supervise employees. Judges exercise authority over litigants. Military officers command units. Public officials govern communities.
Across cultures and throughout recorded history, these relationships have enabled human beings to coordinate responsibilities, develop specialized expertise, allocate resources, transmit knowledge, solve problems, and accomplish collectively what individuals could not accomplish alone.
Human hierarchies are therefore not merely structures of authority. They are among humanity's most powerful structures for collective learning and adaptation.
Yet careful observation reveals a recurring difference in how they function.
Some supervisory relationships encourage questions. Uncertainty can be acknowledged. Mistakes become opportunities for investigation. Existing assumptions remain open to examination. Individuals contribute observations, expertise, disagreement, and alternative explanations, and the hierarchy adapts as understanding improves.
Other supervisory relationships develop very differently. Questions gradually disappear. Existing assumptions become increasingly difficult to challenge. Corrective information may be ignored or suppressed. Individuals learn that disagreement carries risk. Knowledge remains unspoken, problems become increasingly difficult to recognize, and learning slows even when those within the hierarchy sincerely seek good outcomes.
These contrasting patterns appear across institutions that otherwise share remarkably little in common: families, schools, hospitals, businesses, governments, courts, military organizations, scientific institutions, and countless other human hierarchies.
The recurrence of the pattern suggests a scientific question.
Why?
A Deeper Puzzle of Hierarchical Learning
The puzzle becomes more consequential when we consider how learning actually occurs within a hierarchy.
An individual learner generally possesses direct access to many of the resources necessary for learning. An individual can observe a problem, question an existing explanation, draw upon prior experience, devote time to thinking about alternatives, test a new explanation, and modify behavior in response to what is learned.
Human hierarchies are different.
Their extraordinary collective capacity arises precisely because knowledge, expertise, responsibility, authority, and other resources are distributed among different participants.
Those closest to a problem may possess observations, experience, expertise, questions, disagreement, alternative explanations, or corrective information capable of revealing something that others have not recognized. This constitutes a form of frontline intellectual currency: knowledge potentially valuable for recognizing problems and improving understanding.
Yet those possessing this intellectual currency may not control the complementary organizational resources necessary to investigate or act upon it.
Supervisors may control authority, time, funding, priority, personnel, accountability, access to tools and technology, and the capacity for organizational action.
The very structure that gives human hierarchies their extraordinary capacity for specialization and coordinated problem solving therefore creates a dependency that does not exist in the same way for an individual learner:
The resources necessary for learning must somehow be brought back together.
A hierarchy may possess the knowledge necessary to recognize a problem without that knowledge becoming available for organizational problem solving.
It may possess the expertise necessary to develop a better explanation without that expertise being valued.
It may possess both the intellectual and organizational resources necessary to solve a problem while those resources remain separated across an authority relationship.
This reveals a deeper scientific puzzle:
How do human hierarchies continue learning when the resources necessary for learning become structurally distributed across supervisory relationships?
The question is not merely whether hierarchies possess knowledge or resources.
It is whether they can integrate what has become distributed.
Learning Begins with Patterns
Throughout the history of science, discovery has begun with the recognition of recurring patterns.
Ancient astronomers observed predictable movements of celestial bodies before understanding the physical rules governing them.
Naturalists recognized similarities among living organisms before evolutionary theory provided an explanatory mechanism.
Physicians observed recurring patterns of disease before microorganisms were discovered.
Mathematicians recognized numerical relationships before expressing the rules governing them through equations.
Across disciplines, scientific inquiry follows a recurring progression:
A pattern is observed.
A question is asked.
The search begins for the explanatory rule responsible for the pattern.
When a sufficiently accurate rule is discovered, observations that once appeared disconnected become understandable. The pattern may become predictable. In some circumstances, it may also become modifiable.
Human learning follows the same fundamental process.
Learning as the Search for Explanatory Rules
Learning is often described as the acquisition of knowledge.
A more fundamental description is that learning involves discovering increasingly accurate explanatory rules for recurring patterns.
Children discover grammatical rules that allow them to understand and produce language.
Scientists search for rules governing natural phenomena.
Engineers identify principles governing physical systems.
Physicians investigate mechanisms responsible for disease.
Organizations attempt to identify the causes of recurring failures and successes.
Learning therefore involves more than accumulating information.
It involves using observations to develop explanations, comparing those explanations with subsequent observations, recognizing when existing explanations are insufficient, and continually refining understanding.
This process reveals a fundamental requirement for learning:
We must remain capable of discovering that what we currently believe is incomplete or incorrect.
People search for better explanations only when the possibility remains that a better explanation exists.
If we believe we already know the rule responsible for a pattern, we stop looking for it.
Curiosity therefore depends upon uncertainty.
Correction depends upon recognizing the possibility of error.
Continued learning depends upon remaining open to previously unrecognized limitations in current understanding.
This vulnerability exists within every human learner.
Within a hierarchy, however, another problem emerges: the information capable of revealing our error may reside with someone else.
The Structural Vulnerability
The distribution of learning resources is one of the great advantages of human hierarchy.
No physician needs to possess every form of clinical expertise.
No teacher needs to discover independently everything known about education.
No executive can personally observe every process within an organization.
No judge can directly witness every event underlying a dispute.
No government official can personally possess the experience and local knowledge distributed throughout a population.
Specialization allows human beings to know and accomplish collectively far more than any individual could alone.
But specialization and hierarchy also mean that no individual necessarily possesses all of the information required to recognize when his or her understanding is incomplete.
A supervisor may control the resources necessary to act while lacking the frontline observations necessary to recognize the problem.
A subordinate may recognize the problem while lacking the authority or resources necessary to investigate or correct it.
The knowledge exists.
The resources exist.
But they exist in different places.
The adaptive advantage of hierarchy therefore contains within it a corresponding vulnerability:
The same distribution of resources that expands humanity's capacity for collective learning may also separate individuals from the corrective knowledge necessary to recognize limitations in their own understanding.
The question then becomes not simply whether a hierarchy contains knowledge, expertise, and resources.
The question becomes whether its supervisory relationships allow those distributed resources to become available to one another.
Existing Explanations
The recurring supervisory patterns observed across human hierarchies have attracted scholarly attention for generations.
Leadership researchers have examined differences in supervisory and leadership styles.
Organizational scholars have investigated culture, communication, incentives, psychological safety, organizational learning, and systems thinking.
Psychologists have studied personality, motivation, cognition, bias, and decision-making.
Political scientists have examined authority, legitimacy, institutional design, authoritarianism, and democratic governance.
Educational researchers have investigated reflective practice, learner autonomy, intellectual humility, and collaborative inquiry.
Healthcare scholars have examined teamwork, professional hierarchy, patient safety, and clinical learning.
Each of these traditions has contributed important insights into particular dimensions of hierarchical behavior.
Collectively, they have transformed our understanding of human organizations.
Yet the recurring pattern remains striking.
Institutions with profoundly different missions, structures, professional traditions, cultures, and systems of governance can develop remarkably similar supervisory environments.
Some preserve questioning, transparency, collaborative inquiry, and continued learning.
Others increasingly produce conformity, censorship, coercion, silence, and preservation of existing assumptions.
And beneath those observable differences lies the structural problem:
What determines whether the intellectual resources distributed throughout a hierarchy become effectively integrated with the organizational resources necessary for continued learning?
An Observation from Practice
My interest in this question developed gradually through experiences in college and graduate education, clinical medicine, graduate medical education, organizational leadership, quality improvement, and public health.
Across these settings, I encountered people whose stated intentions were often remarkably similar.
Teachers wanted students to learn.
Physicians wanted patients to receive excellent care.
Managers wanted organizations to improve.
Public officials sought to serve their communities.
Yet the supervisory environments created around those intentions could differ dramatically.
Some supervisors appeared genuinely interested in information they did not already possess. Questions were useful. Disagreement could improve understanding. Expertise held by others was valuable precisely because it might reveal something the supervisor had not recognized.
Others responded differently. Questions could be experienced as challenges. Disagreement became disruptive. Corrective information could lose its value when it conflicted with existing understanding. People lower within the hierarchy learned to adapt accordingly.
The difference could not be explained simply by whether supervisors intended good outcomes.
That observation gradually changed the question.
Rather than asking only why organizations differ, I began asking why similar supervisory belief–behavior systems repeatedly emerge across hierarchies that are otherwise so different.
And beneath that question lay another:
Why do some supervisory relationships enable distributed knowledge and resources to come together for continued learning while others allow them to remain separated?
A Different Kind of Scientific Question
Scientific inquiry advances by searching for explanatory rules capable of accounting for recurring patterns.
The supervisory belief–behavior systems observed across human hierarchies constituted such a pattern.
Existing theories described many characteristics of those systems and identified important influences upon them.
But another level of explanation appeared possible.
What if the recurring patterns were not primarily products of particular professions, institutional cultures, management techniques, or leadership styles?
What if they reflected a more fundamental process operating within supervisory relationships themselves?
The investigation therefore began with two connected questions:
How do human hierarchies continue learning when the resources necessary for learning become structurally distributed across supervisory relationships?
and
What underlying cognitive and epistemological rule gives rise to the recurring supervisory belief–behavior systems that influence whether those distributed resources become available for continued learning?
These questions did not assume that such a rule existed.
They established the object of investigation.
The Beginning of an Investigation
Every scientific investigation begins with observations that existing explanations do not fully resolve.
Here, the observations were interconnected.
Human hierarchies repeatedly produced recognizable supervisory belief–behavior systems across otherwise different settings.
Those hierarchies depended upon learning.
Their capacity for collective learning arose partly from distributing knowledge, expertise, authority, and other resources among different participants.
Yet that same distribution created a dependency: continued learning required resources held by different people to become effectively connected.
The question was therefore larger than leadership.
It concerned the relationship among hierarchical structure, human cognition, supervisory relationships, and learning.
Understanding that relationship required beginning somewhere more fundamental than organizations themselves.
Organizations do not recognize patterns.
Organizations do not develop explanatory rules.
Organizations do not experience certainty or uncertainty.
People do.
Understanding how human beings learn, how they develop explanations of reality, why those explanations can become unconsciously mistaken, and what happens when corrective knowledge is distributed across relationships of authority became the starting point for everything that followed.
The search for the rule had begun.
Continue the Investigation
Chapter One has introduced two interconnected puzzles.
The first is observable: why do remarkably similar supervisory belief–behavior systems repeatedly emerge across diverse human hierarchies?
The second is structural: how do human hierarchies preserve their capacity for continued learning when the resources necessary for learning become distributed among people occupying different positions within relationships of authority?
The next chapter examines how previous generations of scholars have approached these problems and asks what their explanations illuminate—and what remains unexplained.
That investigation will progressively narrow the search from the observable patterns of supervisory behavior toward the deeper structural, cognitive, and epistemological processes that may give rise to them.
